Celebrating Our Partners and Spring Calls-to-Action

The rules for interaction from collective impact create an alignment within complex relationships and sets of activities which, when combined with shared intentionality, causes previously invisible solutions and resources to emerge.

Celebrating Partnerships

From Collective Impact and community collaboration research, partnerships have been shown to improve communication, increase trust and respect, breakdown misconceptions, and develop efficient, effective, and sustainable solutions. First 1,000 Days Sarasota is building partnerships, enhancing connections, and providing networking opportunities for community agencies to learn from one another. See some of the feedback from our end of the year 2020 Partner Survey below.

Partner Spotlight: Sarasota Memorial Health Care System & CenterPlace Health

Benefit Assistance and Navigation is a huge need for families in the community. Sarasota Memorial Health Care System and CenterPlace Health have had some recent process changes to help improve this complex barrier.

Sarasota Memorial Health Care System has some real “behind the scenes heroes” in their Patient Assistance Program helping families with benefit navigation. The program is a great resource for families needing assistance signing up for Medicaid or financial support if they are uninsured. We have recently added their program as a service on Unite Us! Pregnant women planning to deliver at Sarasota Memorial Hospital and/or have recently delivered a baby qualify for this service. Clients can be referred through the Unite Us platform by using the Health Insurance/Benefits sub-type.

CenterPlace Health has hired a new Community Health Worker, Ximena. Ximena will be rounding on CenterPlace Health patients on Sarasota Memorial Hospital’s Mother Baby unit five days a week. Her role is to ensure patients have a follow-up appointment for baby and mom and are connected with available resources in the community. We are thankful to have another kind and knowledgeable professional assisting families in the community.

This group will: identify resources in the community, determine whether there are any capacity concerns or gaps in services, provide tools to help practicing professionals screen and refer for services, and start a countywide campaign to educate the general public on supports available.
