Moving from a maze of resources to a coordinated system of care
Unite Us is the nationally recognized care coordination platform that was selected by First 1000 Days Sarasota to help families and health and social service providers navigate the system. The platform has a closed loop referral system, to ensure families get in for services and a real-time data dashboard to celebrate successes and work through barriers to care in the community.
Unite Us Customer Success Manager, Dr. Amanda Terry, held Strategy Sessions with partners the week of July 13th to demo the platform and explain the registration process. Two additional session will be ng offered in August; click the date to register.
The goal is for all registration forms to be submitted by July 31, so we can launch by end of August.
The Client Navigation workgroup is assisting in the development the First ,1000 Days Sarasota universal screening tool and assistance request form for Unite Us. The meeting is scheduled for Friday, July 24, from 1-2pm. If you have interest in attending and/or being a part of the Client Navigation workgroup please email Chelsea Arnold for the Zoom link.
Big thank you to Sarasota Memorial Healthcare System, who signed the Unite Us contract on behalf of the initiative. Their IT and legal department did a thorough evaluation of the system especially in the areas of patient privacy and informed consent. SMH values their relationships with partner organizations in the community and sees the amazing potential this platform can have for Sarasota County!
New logo and parent portal
A small Taskforce, the Education and Outreach workgroup, and parents in the community have been collaborating with our behavior change marketing consultants, Marketing for Change, to develop our brand and parent website. The workgroup identified key features that should be incorporated in the site. Parent feedback was provided on the logo, tag line, and color schematic for the initiative.
The purpose of the parent portal is four fold:
Provide background information about our initiative
Encourage parents to get involved with our Parent Advisory Committee
Normalize the critical nature of the early years through an evidence based parent education curriculum
Provide a safe place for parents to ask for assistance navigating resources in Sarasota County.
The website will launch with Unite Us at the end of August!
Growing National Partnerships
First 1,000 Days Sarasota has partnered with The Basics Learning Network to use their five evidence based principles in our parent education campaign. The five principles are easy to incorporate into busy schedules and can be used with children of any age. Our goal is to saturate the curriculum countywide so that everywhere parents go they are reminded of these key teaching opportunities for their children.
The initiative was also accepted to be a part of the Nemours Integrator Learning Lab, funded by the Kregse Foundation, early this year. We have been working Nemours healthcare system, and nine other initiatives addressing social determinants of health across the country to collaborate about best practices in the areas of networking, strategic planning, sustainability, and and other Integrator functions.
Plan of Safe Care
The initiative’s Program Coordinator Chelsea Arnold is facilitating the development of the Plan of Safe Care for Sarasota County. By convening professionals in healthcare, mental health, substance use, child welfare and parent leaders with lived experience, our goal is create an efficient, trustworthy, and effective system to improve the health and wellbeing for substance exposed newborns and their families. The goal is to pilot the Sarasota County Plan of Safe Care on the Unite Us platform in early 2021.
We need parents!
The initiative is recruiting parent representatives to sit on our Parent Advisory Committee. We are looking for parent voice to be woven into every aspect of the initiative and recruiting a diverse group of parents in Sarasota county is a top priority. Please spread this flyer to parents at your organization and encourage them to get involved.
There are 14 seats on the committee and we still need 8 more parents. Meetings will be held every other month on Zoom and parents will receive $25 gift cards for attendance.
